Terms and Conditions for Health Services
1. Introduction These terms and conditions outline the rules and regulations for receiving general health services provided by Home Therapy. By booking an appointment or receiving treatment, you agree to be bound by these terms and conditions.
2. Services The Practitioner provides general health services, which may include, but are not limited to:
  • Assessment and diagnosis of health conditions

  • Prescribing medications and treatments
  • Preventative health advice
  • Health screenings and check-ups
  • Referrals to specialists, if necessary
The Practitioner may tailor services to meet the individual needs of each patient as determined during the consultation.
3. Appointments & Cancellations
